FortiGuard Web Filter blocking Unrated

I have made sure that the settings are as follows:

#1 the site URL is in the URL Filter for action of allow

#2 Rating Option: Allow Websites When a Rating Error Occurs is enabled

#3 Rate URLs by Domain and IP Address is unchecked

#4 Rate Images by URL (Blocked images will be replaced with blanks) is checked

The site is still blocked with the FortiGuard Web Filtering page stating that the page was blocked because of violation of your internet policy, but with the above list, and I also pushed through the install wizard, the update to the cluster.  What am I missing? why would it still block it?

the firmware on the cluster is FortiGate 6.0.6 build 0272(GA)
