MFA not registering

MFA not registering

I have followed the instructions to assign the token to myself, i get the qrcode, scan and its matched up in the app but when i add the user definition to the correct user group I get this generic 'entry not found'  this is group matched up with active directory via radius.   Support had me reboot the firewall once but that did not help.   Anyone see something like this?

    To update my progress I
     
    -created a new firewall group, ldap back to active directory for auth.  added myself as user from firewall user definition  this saved fine where existing group had the above error.
     
    - assign myself a token and send activation to app.  scan qr and token is now on phone.
     
    - i launch and sign into vpn client fine but never prompted for token.

    This was resolved by support.  The group was not applied the correct IPv4 Policy.
