Internal disk/flash failed - No such device or address

Hello,

i have two boxes FG-80C with same internal disk/flash issue but not sure if is it hardware problem or bug in firmware.

When I login via SSH a get following messages:

admin's password: Cannot read termcap database; using dumb terminal settings.

# diagnose hardware deviceinfo disk

Device S0           7.5 GB      ref: 0          === cannot get scsi device info ===

# diagnose hardware deviceinfo flash do_scsi_pt: No such device or address

Firmware versions 4.0MR3 Patch 12 and 4.0MR3 Patch 15.

Unfortunately I cannot them reboot easily.

Any ideas what to check or debug?
